Pricing & Insurance

  The initial intake session is $275. 

After intake, standard rates for therapy sessions are: 

• $125 for up to 30 minutes (in the rare event sessions end early)
• $175 for sessions between 31 and 52 minutes
• $200 for sessions between 53 and 60 minutes
• $125 for no-shows or cancellations within 48 hours of the appointment

Due to changes Care Oregon announced in August 2025, I am able to take OHP Care Oregon, Health Share, Columbia, and Jackson Care Connect until Sept. 30, 2025, but I will stop taking those insurances after that date. 

I am currently able, and will remain able to accept, OHP Trillium insurance and OHP Open Card. 

Pursuant to regulations governing Medicaid/OHP insurance, Hazelwood Counseling LLC will not charge clients using OHP to pay for services, for copays or coinsurance, or late cancellations or no-shows.

I take the following commercial insurances:

• Anthem Blue Cross Blue Shield
• Cigna
• Kaiser Northwest Permanente 
• PacificSource
• Providence
• Regence Blue Cross Blue Shield
• Moda 

Please note that while Hazelwood is in-network with these insurers, specific exclusions or other provisions in your policy may apply. I recommend you contact your insurance to get specific information on what your plan will cover. 

Additionally, please note that if you have commercial insurance and you cancel a session with less than 48 hours' notice, you will be charged a $125 late cancellation fee.
